First-Time Patients
Health coverage can change frequently. Once you’ve scheduled your first appointment, our staff needs to collect some important information regarding your insurance carrier. Please make sure you have verified coverage and eligibility requirements with your insurance carrier as well.
Appointment Cancellation
We require 24 hours notice of any change or cancellation of appointments. If you are no longer able to keep your scheduled appointment, please call our office at (231) 853-2519 to cancel. This notice gives us the opportunity to use that time to care for other patients.
